**Requirements to be considered a credentialed provider
- Individual NPI or Group NPI
- Individual or group professional liability coverage
- Credentialed with at least 4 primary insurances in the state they are practicing in
- Provider can complete an application we provide
**Maintenance contracts include but not limited to
- Performing recredentialing and attestations with insurance companies, CAQH and facilities
- Updating and maintaining credentialing files
- Notifications and assistance to provider for expiration dates of license, insurance, and education
